Use the relevant questions rather than collecting every possible detail. Leave unnecessary sensitive information out, explain who will use the answers and share only the client-facing pages. Pages 11-12 are for the photographer, not a questionnaire to send to every client.

Pair preparation with clear agreements

This kit is not a contract, privacy notice or model release. Use the free photography session agreement to discuss services and delivery, and a separate commercial model release when wider image licensing is appropriate. Optional publication permission is not part of answering a preparation questionnaire.
